Please fill all fields in the application form, and provide a clear picture of the basis of your character, as well as an image of any alternate forms they might have. Thank you.

Body Modifications: (This includes any scars)

Occupation: (If a member of the League, please state ‘League Member’ here, or in Mina and Tom’s case, ‘League Leader’)
League Position: (Give a title – i.e. The Rogue for Skinner – and then a brief explanation of sorts)

Home(s): (Any places you live. Don't need to put an address, or even too much detail, just a general description of the place or places that you live.)

History: (No need to write a book, just a general description of where the character has been and what the character has done.)
